Instructions and templates of tables are already provided, but you must fill in the table and text around them per the instructions. Feel free to edit the table, or delete and upload a new one as needed. Upon completion of the project you will have a program implementation and evaluation plan. If you need to plan or evaluate programs in the future, the pieces of this project are the fundamental steps you should take and information you will need to provide.
